All Wales Forum and partners in Monmouthshire have committed to working collaboratively to find out ‘what matters’ for learning disability families moving from home to independent living in the region.
We would like to explore this with you through a conversation we are having on the 28th of January at 10:30am – 12pm.
The conversation will involve parents and carers, professionals, and the wider carer-led network to discuss learning and priorities for transitions and explore what we can do in the future.
From this conversation, we would like to find out –
- What works? What does not? Learning from experiences of transitions
- What is out there? Some stories of emergent practice from across the network
- What is next? Collectively agree what we are going to do next
